🔹 Step 1: Visit the Official
CSD Portal
Go to 👉 https://www.csdindia.gov.in
⚠️ This is the official website
of the Canteen Stores Department (CSD),
Government of India.
🔹 Step 2: Understand the
Process
The CSD
Smart Card (URC card) is not issued directly online —
It is issued through your nearest Unit Run
Canteen (URC), but you can
register online and download the form from the portal.
You’ll need to:
1.
Download the
application form
2.
Fill and verify it
through your Unit or Record Office
3.
Submit it to your
Canteen / Station HQ

🔹 Step 3: Download the
Application Form
1.
On the homepage, scroll down and click “Forms” or “Downloads.”
2.
Download “Application
for CSD Smart Card” (available in PDF format).
o Direct
link (if active): https://csdindia.gov.in/downloads
3.
Print the form.
🔹 Step 4: Fill the
Application Form
Fill the form carefully with:

Personal details
(Name, Rank, Service No, PPO No, DOB)
·
Service
details (Service type, unit, station, etc.)
·
Permanent
address
·
Dependents
(spouse, children, parents if eligible)
·
Aadhaar
and PAN details
·
Mobile
number & email ID
Affix:
·
Recent
passport-size photograph
·
Signatures
of applicant and dependents
🔹 Step 5: Attach
Required Documents
Attach self-attested photocopies of:
1.
Service
Certificate / Discharge Book / PPO
2.
Aadhaar Card
(self & dependents)
3.
PAN Card (self)
4.
Passport-size
photos (self & dependents)
5.
Proof of address
(Aadhaar / Driving Licence / Voter ID)
📎 For widows or
dependents, attach death certificate / relationship proof as applicable.
🔹 Step 6: Get Unit /
Record Office Verification
·
Serving personnel → Get CO/OC Unit signature and stamp.
·
Ex-servicemen → Get verification from Zila Sainik Board or Record Office.
🔹 Step 7: Submit to
Nearest CSD Canteen (URC)
1.
Submit the verified form + documents at your nearest Unit Run Canteen.
2.
They will:
o Verify
details
o Capture
biometric data (photo + signature)
o Send
your application to Central CSD Office
(Mumbai) for card generation.
🔹 Step 8: Collect the
Smart Card
·
Once approved, your CSD Smart Card will be printed and delivered to your URC.
·
You’ll receive an SMS or call
to collect it.
·
Pay the card
issuance fee (approx ₹135) at the canteen if applicable.

you can check status via their local
portal or contact number.
💡 Additional Tips
·
Ensure your name and service details match your records exactly.
·
Use black/blue
ink only on the form.
·
Don’t laminate or alter any document copy.
·
Keep photocopies of everything you submit.
