HOW TO APPLY FOR 64KB ECHS CARD ONLINE


Here’s a complete step-by-step guide to apply online for the new 64 KB ECHS Smart Card (Ex-Servicemen Contributory Health Scheme) 👇


🖥Step-by-Step Process to Apply for 64 KB ECHS Card Online


🔹 Step 1: Open the ECHS Portal

Go to 👉 https://echs.sourceinfosys.com


🔹 Step 2: Register / Login

·         If you are a new user, click “Register Now” and create an account using:

o    PPO Number

o    Date of Birth

o    Mobile Number

o    Email ID

o    Aadhaar details

·         If you already have an account, click “Login” and sign in.


🔹 Step 3: Select “Apply for Smart Card”

After login:

1.      On the dashboard, click “Smart Card Application” or “Apply for 64KB ECHS Card”.

2.      Read the instructions carefully and proceed.


🔹 Step 4: Fill in Application Details

You will need to enter:

·         Personal details (Name, DOB, Aadhaar, etc.)

·         Service details (Service No., Rank, PPO No., Record Office, etc.)

·         Dependents’ details (spouse, children, parents if eligible)

·         Upload photos and documents (see below)


🔹 Step 5: Upload Required Documents (in PDF/JPEG format)

You’ll need to upload clear scanned copies of:

1.      PPO (first and last page)

2.      Discharge Book / Service Book

3.      Address Proof (Aadhaar / Driving Licence / Voter ID)

4.      Photograph and Signature (for each member)

5.      Dependent Documents:

o    Spouse: Aadhaar & photo

o    Children: Birth certificate / Aadhaar

o    Parents (if claimed): Income certificate + Aadhaar

⚠️ Ensure file size is under 1 MB per document.


🔹 Step 6: Verify and Submit

1.      Review all entered details carefully.

2.      Click “Submit Application”.

3.      Note the Application Number — you’ll need it for tracking.


🔹 Step 7: Payment of Card Fee

After submission:

1.      You’ll be redirected to online payment (₹177/- per card approx).

2.      Pay via debit card, credit card, or net banking.

3.      After successful payment, download the Payment Receipt.


🔹 Step 8: Print & Submit Hard Copy

1.      Print the Acknowledgement Form and Payment Receipt.

2.      Attach a passport-size photo of each applicant (self & dependents).

3.      Sign the form.

4.      Submit it to your nearest ECHS Polyclinic / Station HQ for verification.

🔹 Step 10: Card Printing & Collection

Once approved and printed:

·         You’ll receive an SMS from ECHS.

·         Ensure all dependents have valid Aadhaar numbers.

·         Photographs must be recent (white background preferred).

·         Applications with incomplete or unclear documents may get rejected.
